| County Health Rankings: Mobilizing Action Toward Community Health (MATCH) |

The County Health Rankings, a first-of-its-kind collection of 50 reports – one per state —demonstrate that the health of a community depends on many different factors, ranging from individual health behaviors, education and jobs, to quality of health care, to the environment. The Robert Wood Johnson Foundation collaborated with the University of Wisconsin Population Health Institute, which has ranked the health of Wisconsin’s counties since 2003, to develop the Rankings for each state’s counties. This model has been used to rank the health of counties in Wisconsin for the past six years and was built on the work of America’s Health Rankings, ™ which has provided reliable, state-specific rankings of population health for 20 years. The Rankings are a “call to action” for communities to work together to develop programs and policies that address the multiple influences on health. For the first time, public health and community leaders, policy-makers, consumers and others can see how healthy their county is, compare it with others within their state and find ways to improve the health of their community.
